Synopsis
Many healthcare providers in BC encourage and support capable adults to reflect upon and express their instructions, wishes and thoughts about their health care in advance of a health crisis. The laws in BC provide the option of placing these instructions and wishes in legal documents such as Representation Agreements and Advance Directives. Lawyers generally obtain information from their client’s, give advice and reduce the client’s instructions into the appropriate legal document. When the health care providers receive the legal documents, all too often it is difficult to know whether the client/patient fully understood the treatment that they have stated they want or decline or whether the Representatives that they appointed know what the client/patient wanted.
Cari Hoffmann and Dr. Charlie Chen will discuss the practical application of Representation Agreements and Advance Directives and how lawyers and the health professions are able to work together to support the advance planning processes.
Speaker Profile
Cari Borenko Hoffman has supported, and facilitated, the development of disease and program specific advance care planning education with Fraser Health’s Renal, Residential Care and Assisted Living, Older Adult, Mental Health and Substance Use and Cardiac programs. She is the chair of the National Community of Practice, Educators Canada and a member of the National Task Group. Cari has provided support to clinicians and organizations around the world. Professionals from Canada to New Zealand, to Singapore and Japan have benefited from Cari’s expertise; and adapted their advance care planning programs and materials as a result.
Cari is the provincial clinical lead for advance care planning with the B.C. Centre for Palliative Care and is a clinical instructor at UBC Department of Medicine.
Dr. Charlie Chen is a palliative care consultant working at the Royal Columbian Hospital and in the New Westminster community. He is also a Clinical Assistant Professor in UBC’s Department of Medicine. He is originally trained as a family physician and underwent a six month palliative care fellowship. Having spent many years as a half time family doctor and half time palliative care consultant, he now devotes full time to palliative care as a clinician, educator, and advocate. He completed a Masters of Education degree in 2014. A leader in Advance Care Planning, he sits on Provincial and National task forces to promote patient-centred care. Along with his duties as the Program Director for the palliative medicine residents, he is also the physician education lead for the End of Life Care Program at Fraser Health.
